{"id":2049,"date":"2018-11-08T16:33:11","date_gmt":"2018-11-08T16:33:11","guid":{"rendered":"http:\/\/blogs.kent.ac.uk\/biosciences\/?p=2049"},"modified":"2018-11-22T16:01:04","modified_gmt":"2018-11-22T16:01:04","slug":"diagnosing-sleeping-sickness-with-a-1-test","status":"publish","type":"post","link":"https:\/\/blogs.kent.ac.uk\/biosciences\/2018\/11\/08\/diagnosing-sleeping-sickness-with-a-1-test\/","title":{"rendered":"Diagnosing sleeping sickness with a \u00a31 test"},"content":{"rendered":"<p>Researchers in the <a href=\"https:\/\/www.kent.ac.uk\/bio\/\">School of Biosciences<\/a> have developed a simple and cost-effective test to diagnose sleeping sickness, a disease widespread in sub-Saharan Africa. The test removes the need for extensive infrastructure such as electricity, refrigeration and large teams of skilled people that can hinder access to diagnosis and treatment.<\/p>\n<p>The blood spot tests, which can be manufactured for as little as \u00a31, are simple to use and give results in 15 minutes. This enables those carrying the disease but who are not yet symptomatic to be diagnosed and treated promptly, preventing both disease progression and the individual from infecting others.<\/p>\n<p>Lead researcher Dr Barrie Rooney identified the need for a quicker, simpler diagnostic method after spending 10 years working in the field for <a href=\"https:\/\/www.msf.org.uk\/\">M\u00e9decins Sans Fronti\u00e8res<\/a> (MSF)<em>. <\/em>She says: \u2018Particularly in developing countries where people have to travel miles each day for food, water and healthcare or are displaced by war, there is a need to diagnose and treat infectious diseases where and as they occur, rather than letting them develop into epidemics.<\/p>\n<p>Sleeping sickness \u2013 human African trypanosomiasis \u2013 is caused by the parasite <em>Trypanosoma brucei gambiense<\/em>, which is carried by the tsetse fly. Other diagnostic methods not only require large amounts of equipment and expertise, but also involve painful and time-consuming procedures such as <a href=\"https:\/\/www.nhs.uk\/conditions\/lumbar-puncture\/\">lumbar punctures<\/a>. Diagnosis can take weeks, during which the infection can spread to hundreds of people.<\/p>\n<p><a href=\"http:\/\/blogs.kent.ac.uk\/biosciences\/2018\/11\/08\/diagnosing-sleeping-sickness-with-a-1-test\/3865-3x2-br\/\" rel=\"attachment wp-att-2075\"><img loading=\"lazy\" class=\"alignleft size-medium wp-image-2075\" src=\"http:\/\/blogs.kent.ac.uk\/biosciences\/files\/2018\/11\/3865-3x2-BR-300x200.jpg\" alt=\"\" width=\"300\" height=\"200\" srcset=\"https:\/\/blogs.kent.ac.uk\/biosciences\/files\/2018\/11\/3865-3x2-BR-300x200.jpg 300w, https:\/\/blogs.kent.ac.uk\/biosciences\/files\/2018\/11\/3865-3x2-BR-768x512.jpg 768w, https:\/\/blogs.kent.ac.uk\/biosciences\/files\/2018\/11\/3865-3x2-BR-1024x683.jpg 1024w, https:\/\/blogs.kent.ac.uk\/biosciences\/files\/2018\/11\/3865-3x2-BR.jpg 1920w\" sizes=\"(max-width: 300px) 100vw, 300px\" \/><\/a><\/p>\n<p>Dr Rooney first approached <a href=\"https:\/\/www.kent.ac.uk\/bio\/profiles\/staff\/smales.html\">Professor Mark Smales<\/a> at the University in 2012 with the idea for the test. With help from <a href=\"https:\/\/bbsrc.ukri.org\/funding\/filter\/flexible-interchange-programme\/\">a BBSRC FLIP grant<\/a>, she made the move from the field to the laboratory and soon identified three antigens, or markers, that could recognise the sleeping sickness parasite in a blood sample from someone carrying the disease. Crucially, the researchers produced recombinant antigens \u2013 genetically engineered protein molecules \u2013 from an organism closely related to the sleeping sickness parasite, rather than using the sleeping sickness parasite itself. This reduces the cost and also prevents test manufacturers from being infected with sleeping sickness.<\/p>\n<p>Sleeping sickness is classified as a Neglected Tropical Disease (NTD) by the World Health Organisation (WHO). This means that it has little investment by pharmaceutical or diagnostic companies. However efforts by researchers such as Dr Rooney and organisations like MSF have substantially reduced numbers of cases with 2,804 new cases reported in 2015. This is the lowest level since the start of systematic global data-collection 76 years ago. However it is still a threat to millions of people in 36 countries and without prompt diagnosis and treatment, it is nearly always fatal.<\/p>\n<p>Dr Rooney says: \u2018A big problem with sleeping sickness is that it can lie dormant for years, and yet, asymptomatic people can still pass on the disease. Indeed, recently a case came to light in which a 62-year-old gentleman presented at a hospital in Wales with symptoms that turned out to be the disease. He had last been in an endemic country 29 years ago.\u2019<\/p>\n<p>The test is currently undergoing clinical trials in the field. Laboratory results have shown that it has a 98 per cent sensitivity (able to identify 98 per cent of patients with the disease) and 99 per cent specificity (able to correctly identify patients without the disease).<\/p>\n<p>Dr Rooney has established <a href=\"http:\/\/www.trozonx.com\/\">Trozon X17<\/a> \u2013 a not-for-profit organisation \u2013 to bring the test fully to market and to develop tests for other NTDs. She says: \u2018The aim is to develop tests that are robust enough to stand up to tropical climates, but simple enough to be used by non-specialists.\u2019<\/p>\n<p>Dr Rooney is currently developing a test for <a href=\"http:\/\/www.who.int\/news-room\/fact-sheets\/detail\/chagas-disease-(american-trypanosomiasis)\">Chagas disease<\/a>, another parasitic disease, which currently infects around eight million people in Latin Americas. Because the disease causes nervous system, digestive and heart problems, 40 per cent of those infected will die within 10-30 years.
